Emily started her nursing career as an operating room nurse in Philadelphia. She completed her Family Nurse Practitioner training at the University of Pennsylvania and then relocated to Denver with her husband and their dog, Birdie. Emily began working in primary care in Denver upon graduating and quickly realized that the current fee-for-service model provided sick care, not healthcare. Emily believes in creating an open and collaborative environment, aiming to support her patients in informed decision-making and encouraging patients to advocate for their healthcare needs. Emily’s goal is to make you feel comfortable, heard and empowered. Emily wants each patient to feel like family.

What is Direct Primary Care?

DPC is an innovative approach to healthcare that revolutionizes the way that individuals, families, and employees can be taken care of. There is a simple monthly membership plan that gives you direct and unlimited access to high-quality and affordable healthcare without additional co-pays. Your membership covers up to 90% of your, your family’s, or employees' healthcare needs.

Are long term contracts required?

There are NO long term contracts. The patient agreement is on a month to month basis. Of course we want to keep you happy and healthy as a patient at Cultivate Primary Care and we hope to build a long relationship with you. If you find we are not the right fit, you can cancel with a 30 day written notice.

How does DPC work if I have insurance?

We encourage all of our patients to have some kind of catastrophic coverage for issues that are outside our scope of practice (emergencies, surgeries, specialist treatment). DPC works incredibly well with high deductible plans, health shares, and catastrophic coverage plans. You may also choose to use your insurance for labs, imaging and specialist referrals. 

If I have insurance, will it cover my membership fee?

Typical insurance polices will NOT cover membership fees. Many health cost sharing policies will provide a discount on your premium if you are a member of DPC. Check out Zion Health Share for more affordable healthcare options.

How do we offer the extended time and care for our patients?

The typical primary care provider in today's medical system has 2000-3000 patients with visits lasting 15-20 minutes. Our providers are limited to apparoximately 300 patients to ensure individualized care for each and every one of them with visit lengths 30 minutes to 1 hour. 

What if I have an urgent medical need outside of normal business hours?

Members must call the office and leave a voicemail on the urgent line and a provider will get back to them as quickly as possible.

What if my provider goes on vacation?

All members will be notified via e-mail if your provider plans to be out of the office for more than 3 days. There will always be a covering provider.

How do I cancel my membership?

Although we want all of our members to stay forever, we realize that sometimes life events get in the way and takes you on a different path. We require a 30 day notice to cancel your membership. To start the process of cancellation you can email, call, or text the office and let us know you are wishing to cancel your membership and we will assist you in this process.

Please note, should you choose to rejoin, we are not able to guarantee you will be able to have the same provider and we do have a $200 re-enrollment fee that will be applied.
